Consent to Osteopathic Treatment

During your osteopathic consultation you will be asked questions and have a physical examination.

Osteopathic care is recognised as being an effective and safe method of care for many conditions.

I understand the results are not guaranteed. I understand and am informed that as in the practice of medicine, in the practice of Osteopathy, there are risks to osteopathic treatment included but not limited to muscle and joint soreness, sprains, and an exacerbation and/or aggravation of my presenting complaint of an underlying condition.

I have read and understood the above and will ask any questions I have relating to this consent. I intend this consent form to cover the entire course of treatment for my presenting condition, and for any future conditions that I may seek treatment for, with Cate McDonald.

I understand that I can withdraw my consent at any time without prejudice.

I consent to receive osteopathic treatment and understand I can withdraw my consent at any time .
